Superficially there is obviously the Dominant and the submissive, but it's more complex in that there are agreements and safewords, so in reality the submissive is usually the one really in charge - usually. It's about previous abuse, fantasies and trust. it also often transcends the usual gender stuff. It can of course go horribly wrong too.

Consensual non-consent of some form or another usually makes up the top of the lists of folks sexual fantasies. There is something about the fantasy of handing over control I suppose. Probably similar to the thrill people get from other objectively safe activities - riding rollercoasters and so on.
